What is 5 Whys

The “5 Whys” method is a problem-solving technique that aims to identify the root cause of an issue by repeatedly asking “why.”

It helps to uncover underlying factors contributing to a problem. It is important not to settle for the first answer and to push for deeper understanding.

Blaming individuals is not an acceptable answer; instead, focus should be on process improvement.

Prioritize one or two answers for further investigation and stop when causes are beyond control. The method should be used to ask questions, not propose solutions.
